GENERAL DESCRIPTION OF CLASS
The REGISTERED NURSE 1 assumes primary responsibility for and provides direct or Indirect professional nursing care to patients having physical and/or mental disorders. The Registered Nurse 1 assesses patients' needs, develops nursing care plans, implements nursing care actions, and evaluates care; may provide education and consultation to staff, students, patients, and the community.
This is the first of a two-level series. This class is distinguished from the higher level by the absence of responsibility for assisting the Department Director in the management of the patient care area/environment and the absence of assuming Department Director functions in the absence of the Department Director.
DUTIES AND RESPONSIBILITIES
1. Preparation of Nursing Care Plans. Typical tasks: assesses patient's psychological and physical condition, together with other health care professionals; assesses treatment needs and develops nursing care plan including such things as physical care, psychological care, teaching plan and discharge plan; collaborates with others in providing client services; prioritizes and carries out therapeutic treatment prescribed by the nursing care plan and evaluates response to treatment.
2. Direct or Indirect Patient Care. Typical tasks: prepares appropriate equipment for patient care (l.e. IV's, monitors); assesses patient's physical and emotional condition and initiates interventions according to identified nursing care priorities; collaborates with others in providing client services; administers medical and nursing regimens including medication, treatments, dressings, IV's, traction, assisting with surgery, monitor log and evaluating vital signs, placing a variety of catheters and suctioning; may assess potential transplant donors and make "accept or reject" decision; operates and monitors sophisticated medical equipment; assists the patient in activities of daily living; evaluates effectiveness of care and adjusts accordingly; initiates cardiopulmonary resuscitation or other emergency response activities; evaluates and carries out physician's orders; coordinates all patient health services by communicating with nurses, doctors, pharmacy, laboratories, physical therapy, occupational therapy, respiratory therapy, dietary, and social services; may have some security responsibility over mentally 111 patients; responds to calls from the general public and
3. Documentation/Agency Duties. Typical tasks: keeps accurate legal written records through charting, nursing care plans, and case records of such things as: patients' health status and any changes; treatments; medications administered; unusual incidents involving patients and patient education; all data pertaining to surgery such as time patient entered operating room, time operation begins. position of patient during surgery, diagnosis, supply counts for billing purposes; objective data such as patient requests or instructions to nursing staff on following shift; laboratory data, known allergies; height; weight; vital signs; documentation of preoperative care and procedures; may attend and participate in meetings with State and county health officials and Department Directors, physicians, staff to discuss current issues; reviews and audits agency and departmental techniques and procedures being followed.
4. Education. Typical tasks: educates patients and their families about their Illness or disease process, pre and post operative process and orientation to the hospital ward, operating room or birthing room; educates patients and/or their families in posthospital care such as how to bathe, clean wounds and change dressings, feedings, suction and care for tracheotomies, care for sutures and catheters, taking vital signs and glucose levels, mobility, skin care, pain control, medication administration such as insulin or antibiotics, operation of home monitors, CPR for parents of premature babies, parenting skills, breast feeding, contraception, nutrition, and diet; participates in providing learning experiences for nursing students and students from other health disciplines when appropriate; facilitates orientation of new nursing staff; may give presentations in staff meetings or in-service training sessions on new or specialized procedures or health care techniques; may speak to hospitals, community groups, health care agencies, church groups and schools to provide education on current practices, procedures, medical breakthroughs, and the Oregon Donor Program.
5. Charge Responsibilities. Typical tasks: on a designated basis, makes patient assignments to nurses and other health care staff matching the level of skill with the Illness of the patient and distributing the patient caseloads as evenly as possible; observes staff performance; resolves interpersonal problems between staff; assigns new patients to rooms, adjusts staff according to changing workloads; projects staffing needs for oncoming shift; verifies hours for payroll; transcribes and verifies physicians' orders.
RELATIONSHIPS WITH OTHERS
Employees in this class are in regular contact in person and on the telephone with patients to provide care and education and with patients' families to provide support and answer questions regarding patient's condition and medical procedures. Employees are in regular contact with physicians to clarify chart orders, report changes in patient's condition, and collaborate on patient care, and with residents and student nurses to provide education and share information about patients and health care procedures. Employees are in regular telephone and in person contact with support services such as pharmacy, dietitians, x-ray, patient transportation, respiratory therapy, occupational therapy, physical therapy, laboratories, and outpatient social services to provide ancillary services to patients. May share information with community groups, health caregivers, church groups, and schools.
Employees in this class receive general supervision from an Administrative Coordinator and Department Director who review work thorough nursing care plans, direct observation, program results, performance standards and feedback from patients, patients' families and physicians. Work performed by employees in this class is governed by Federal and State laws, hospital accrediting bodies, policies and procedures of the agency and practices of the nursing profession.
